In this role, you will be tasked with implementing physical therapy interventions tailored to individual patient needs, derived from detailed assessments and coordinated with a multidisciplinary team. Your expertise will directly contribute to enhancing patient mobility, alleviating pain, and substantially improving their quality of life post-acute care phases. Conduct comprehensive patient assessments to determine mobility, strength, and coordination issues Design and implement personalized rehabilitation programs Collaborate with healthcare professionals to coordinate treatment plans Monitor patient progress and adjust treatment plans as needed Educate patients and families on treatments and home exercise programs Ensure compliance with all healthcare regulations and maintain accurate patient records Participate in continual education to remain abreast of industry practices Proven ability to design effective patient therapy programs Strong interpersonal skills for interacting with patients, families, and medical staff Excellent time management and organizational skills Capability to handle physically demanding tasks Proficient in using physical therapy software and equipment Medical Insurance Dental Insurance Vision Insurance Current state license to practice physical therapy CPR certification preferred
