In this role, you will provide support to Nurse Practitioners and
patients in long term care settings; support local markets in
launching new facilities; provide coverage to local market Nurse
Practitioners; deliver high quality care on-site. This is a
flexible, autonomous role that creates enormous satisfaction for
the NP as you impact the care and comfort of our aging population.
All the while, you'll be building meaningful relationships with the
residents and their families and the health care providers who are
responsible for their care.
This position requires travel through the state of New York
downstate area and New Jersey. Travel between care sites is based
on the needs of business/market and required for the role. Expected
Travel 75% - 100%

New York City, NY Residents Only: The salary range for New York
City residents is $90,500 to $161,600. Pay is based on several
factors including but not limited to education, work experience,
certifications, etc. In addition to your salary, UnitedHealth Group
offers benefits such as, a comprehensive benefits package,
incentive and recognition programs, equity stock purchase and 401k
contribution (all benefits are subject to eligibility

PLEASE NOTE Employees must be in an active regular status.
Employees must remain in role for a minimum of 12 months from the
date of hire /rehire/transfer. If an employee leaves Home and
Community, the student loan repayments will cease. The employee
must remain in an Advanced Practice Clinician or Physician role
within Home and Community for 36 months to receive the full benefit
of the student loan repayments.
