Job Description:

We are looking for a motivated, hardworking, and experienced Nurse Practitioner or Physican Assistant candidate to join the growing Visiting Docs team.
The ideal candidate would have previous experience working with patients in their residences or in assisted living facilities. -
Responsibilities include but are not limited to: -
--- Providing general primary care to facility patients.
--- Diagnosing, treating and managing both acute and chronic conditions.
--- Prescribing and refilling medications as appropriate.
--- Actively communicating with patients, families, other specialists and facility personnel (case managers, social workers, med room, etc).
--- Ordering and interpreting diagnostic tests such as imaging and lab work.
--- Documenting patients' histories and treatments in EMR.
--- Seeing patients via telehealth, as needed.
Qualifications:
--- Active unrestricted New York state NP (must have board certification) or PA license.
--- Minimum three (3) years of experience preferred.
--- PECOS enrolled.
--- DEA certified.
--- BCLS.
--- Knowledge of eClinicalworks preferred.
Part time schedule with no evenings or weekends required. - Ideally schedule would be Monday-Friday 9:00 AM - 2:00 PM.
