This organization has a leadership team of visionaries among the best in the business and has been recognized as one of the premier Senior Living and Care Organizations in NYS.This organization is in a growth phase and is looking for an experienced Clinical Care Manager for their facility in Rye, NY:

Position Overview:
The Clinical Care Manager (CCM) is responsible for the day-to-day coordination and oversight
of all aspects of the Nursing Department, for his/her assigned floor, in accordance with current
Federal and State regulations, as well as local regulations as assigned by the Director of

Nursing. The Clinical Care Manager plans and provides managerial and/or clinical support to

patients and staff within the Nursing department for health care, long term care, and short term
rehabilitation.

Essential Job Functions:

• 
  
• Assists in ensuring nursing staff is providing quality and appropriate resident/patient carethat meets or exceeds company and regulatory standards.
  
• May assist with recruiting, hiring, providing orientation/training, and retaining a staff tocarry out the responsibilities of nursing service.
  
• Works closely with physicians, nurse practitioner, Director of Nursing, andinterdisciplinary team in communicating pertinent information regarding resident condition, and ensuring that physician orders are carried out in a timely fashion.
  
• Assists in maintaining administrative authority, responsibility, and accountability for the
  proper charting and documentation of care and of medications and treatments
  
  Qualifications:
  
  
  • 
    
  • 3+ years of Nurse Management experience preferred
    
  • NYS RN License, BLS/ACLS certification (required)
    
  • Comprehensive knowledge of nursing service policies and procedures
    
  • Must possess leadership and supervisory ability, excellent communication and team building skills
