Works to identify new, emerging and promising practices that improve the efficiency, effectiveness and productivity of clinical workflows across the healthcare continuum at Brookdale and to help bring those practices to scale. Oversees implementation of clinical innovation initiatives and provides guidance and direction to divisional, regional and district associates. Identifies performance improvement opportunities, socializes them among key stakeholders, and creates processes to operationalize. This includes helping to assess feasibility and potential ROI for proposed innovative solutions (i.e. advancements in telemedicine).
        • Serves as clinical lead on value-based care programs contributing to organization wide strategy development, mission achievement, and cross-departmental collaboration (i.e. clinical, operations, quality, reporting, IT systems, legal and contracting teams)
        • Engages key stakeholders in program development to ensure effective implementation, compliance, and contractual arrangements in order to fulfill regulatory requirements.
        • Develops and monitors deadlines, budgets, and other activities; leads clinical innovation and strategy projects to meet assigned goals and deliverables; provides project status updates to stakeholder groups and upper level leadership.
        • Supports the HealthPlus program by creating frameworks and processes to operationalize population health and value based care arrangements to execute complex business strategies and proactively drive results.
        • Provides clinical thought leadership and strategic direction to help advance Brookdale priorities
        • Initiates and facilitates a cross continuum clinical advisory group to help inform and guide the clinical innovation strategy
        • Develops trusting relationships with clinical leaders, department leaders, referral partner organizations and executives.
        • Identifies new projects, partners, and opportunities to maximize mission impact and drive alignment with the organization's strategic objectives
        • Fosters a culture of clinical innovation, continuous improvement and creativity to enable Brookdale to respond to the rapidly evolving healthcare landscape and priorities.
        • Provides consultation to internal teams on population health topics and build external relationships with community partners.
        • Completes special projects and assignments as requested. This job description represents an overview of the responsibilities for the above referenced position.           Bachelor's degree in Nursing, Business, Health Care Administration, Public Health, or other health related degree. Experience leading change management and performance improvement within healthcare organizations. Experience in implementation and support of healthcare innovation. Direct knowledge, understanding and experience in Value-Based Healthcare, Population Health Strategies and Health information technology. 7+ years in healthcare related arena or experience in a healthcare industry related environment. Experience from within healthcare insurance provider, hospital administration/management preferred. Demonstration of experience with progressive leadership in a large complex healthcare system, serving multi-disciplinary customers preferred.Required Skills, Knowledge, and Abilities
          • Knowledge of health systems operations, health policy and/or health care contracting; general knowledge of healthcare business processes and the associated drivers
          • Knowledge of population health programs and concepts; demonstrated ability to develop, implement and evaluate population health programs
          • Experience with the delivery of Value Based Care initiatives.
          • Expert team management and facilitation skills; ability to motivate and educate other team members
          • Demonstrated project and portfolio management skills; advanced written and verbal communication skills
          • Demonstrated ability to lead diverse teams to achieve outcomes, as well as form and maintain collaborative relationships
          • Ability to identify data needed for both assessment and decision-making purposes
          • Ability to deploy appropriate data sources, analysis methods and effective communication of analytical results
          • A self-motivated, independent, highly driven and results focused individual.
          • Ability to analyze and resolve complex logical issues.
          • Attention to detail, professional demeanor, thorough and logical approach.
          • Ability to leverage best practices, analytics, and data-driven processes to develop new and innovative value-based care products.Management/Decision Making
