Be a part of our team of healthcare heroes and discover why Catholic Health was named Long Island's Top Workplace.Our Care Manager (RN) will:Assess, plan, implement, monitor and evaluate options and services to effect an appropriate individualized plan of care for patients across the acute care continuum. Perform required activities for utilization management, PRI assessment and completion, transition planning, and identification of actual or potential quality/risk indicators, in compliance with regulations and organizational goals. Perform as an integral part of the interdisciplinary team to effectively ensure optimal patient outcomes and length of stay efficiency. Consider payer requirements and respond accordingly to avoid adverse financial consequences to patients and the hospital. Work proactively with ED staff, ED charge nurse, and ED Physicians to identify potential admissions and their plan of care.Determine DRG-based initial LOS values for ED admissions (all status/types).Be required to pursue ongoing education, certification and self-development to remain current with industry standards and business objectives related to Care Management as appropriate.Have knowledge of The Joint Commission/CARF (as appropriate) NCQA (as appropriate) standards, and Federal, State and PRO regulations preferred.Partner with an interdisciplinary team including, but not limited to, attending physicians, Registered Nurses (RNs), and ancillary staff to optimize patient outcomes and satisfaction.Advocate for and build meaningful relationships with patients and their loved ones through safety intervention, emotional support, and education.What you'll bring:Passion for Care Management 3-5+ Years acute care clinical RN experience 2-3+ Years Utilization Management, Performance Improvement, Transition Planning and/or Care Management experience in an acute hospital or related managed care settingEducation: Graduate of an accredited School of Nursing requiredBachelors of Science in Nursing (BSN) degree strongly preferredLicensure: New York Registered Nurse (RN) License & RegistrationCertification: Basic Life Support (BLS) certification is required.
