Time Off and more! Job Summary: CompassCare is seeking a full-time
Director of Clinical Services to lead our Clinical Team. This
talented and skilled individual will be responsible for clinically
assessing clients and their home environments, developing each
individualized client care plan, and providing and coordinating
necessary clinical services to provide exceptional care for each
client. This clinical and administrative position is primarily
based out of our Stamford CT headquarters and requires local travel
and virtual visits within CompassCare's territories to ensure the
overall implementation, delivery, and coordination of CompassCare's

start-up company that is growing. Responsibilities:
- Overall administration and development of necessary policies
and procedures for the provision of clinical support to clients and
caregivers;
- Identify individual client needs in order to provide
high-quality care and perform initial and periodic onsite client
assessments.
- Develop and maintain client care plans, working in partnership
with other healthcare professionals on the client's clinical team
(e.g. physicians, physical therapists, occupational therapists,
discharge planners, social workers, and other health care
professionals as needed);
- Support and coordinate clinical care for clients, including but
not limited to medication management and oversight as needed;
- Collaborate with the Company's Training Manager to ensure
proper instruction to caregivers regarding each client's needs and
care plan;
- Collaborate with the Company's Training Manager to maintain and
further develop in-house clinical education and training materials
for caregivers;
- Oversee development and coordination of part time and per diem
RNs on staff;
- Ensure quality control and assurance of direct patient
care;
- Support and advise caregivers, clients and their families on
clinical issues;
- Communicate and coordinate with client's outside clinical
team;
- Other related duties and responsibilities as assigned. Desired

skills.
- Strong interpersonal skills for developing relationships with
clients, their families and our caregivers.
- Extensive hands-on clinical experience.
- Ability to work with a variety of health care professionals,
clients and caregivers.
- Familiarity and working knowledge of advanced directives
- Commensurate technical skills and ability to multi-task.
Preferred:
- 5+ years of experience as a Clinical Manager, Coordinator of
Care or RN Supervisor with a home care or home healthcare company
or in an institutional setting (i.e. assisted living, skilled
nursing facility, hospital).
- Experience working with older adults and their associated
comorbidities. Qualifications:
- Bachelor's degree in Nursing, Social Work, or a related
field
- Registered Nurse ("RN") licensed to practice in New York and/or
Connecticut as specified by each state's regulations.
- Minimum 2+ years of experience as an RN
- Experience in direct patient care and assessments/observation
of clinical conditions; with strong clinical skills.
- Knowledge of age-related illnesses, including understanding of
Palliative & Hospice Care.
- Excellent verbal, written, and communication skills
- Must be able to work flexible hours, including some evening and
weekend hours.
- Ability to work onsite in the corporate office in Stamford, CT,
as well as ability to travel throughout CompassCare's area of
service.
